Original valid passport of student + photocopy  
School transfer certificate from previous school attested if student is transferring from another school             Latest Report Card                                                                                                                                                             Birth Certificate + photocopy                                                                                                                          Immunization Card  + photocopy                                                                                                                                        OHIP (Ontario Health Insurance Plan) card  +  photocopy

Important remarks:

The student will not be officially registered unless all documents required are submitted.  
English is the official language accepted in official documents, otherwise documents must be translated by a legal translator and authenticated.
